This form is to be used by motor vehicle, mobile home and recreational vehicle dealers. It is very important to notify the Division of any major changes, especially changes in location, a major decision in every business. Please remember that compliance examiners may contact you periodically. A business that changes locations may also change phone numbers, fax numbers, etc. The division will attempt to contact a dealer, either by phone, electronically, or by mail. Once the Division determines that there seems to be no evidence of business activity, a request to terminate the license may be processed. The dealer may not have intended to go out of business, but without proper notification, the Division may have no other alternative.
