The first attachment HSMV86056: http://www.flhsmv.gov/dmv/forms/BFO/86056.pdf is the State of Florida Application for a Dealer license. The application itself has valuable information so read it carefully. Additional documents and information that needs to be submitted along with the application form.

A completed application form HSMV 86056 and $300 fee for motor vehicle dealers or $340 fee for mobile home or recreational vehicle dealers;
  • Proof of identification/DL
  • A true copy of property lease or proof of ownership;
  • Proof of electronic fingerprints (Receipt) for each new officer from a FDLE authorized service provider.
  • Copy of dealer training course certificate;
  • Original surety bond/irrevocable letter of credit.
  • Copy of business and/or fictitious name registration (if fictitious name is applicable) (on-line http://www.sunbiz.org/);
  • Copy of certificate from the Division of Corporations showing current registration of business to conduct business in the State of Florida (on-line http://www.sunbiz.org/);
  • Corporation papers, corporate meeting minutes showing the election of corporate directors, Article of Incorporation if the dealership is a Corporation or Incorporation; Articles of Organization and Operating Agreement if the dealership is a Limited Liability Company; or a Partnership Agreement if the dealership is a Partnership;
  • Independent dealers (VI), Auction dealers (VA), and Wholesale dealers (VW) have the option to submit a copy of: a garage liability insurance certificate which shall include, at a minimum, $25,000 combined single-limit liability coverage including bodily injury and property damage protection and $10,000 personal injury protection; OR a general liability insurance policy coupled with a business automobile policy, which shall include, at a minimum, $25,000 combined single-limit liability coverage including bodily injury and property damage protection and $10,000 personal injury.
  • Salvage dealers (SD) are exempt from submitting a garage liability insurance coverage.
  • Franchise dealers (VF), Recreational dealers (RV & RU), and Mobile Home dealers/brokers (DH/BH) selling recreational vehicles must submit a garage liability insurance certificate which shall include, at a minimum, $25,000 combined single-limit liability coverage including bodily injury and property damage protection and $10,000 personal injury protection.
  • Federal Employer’s Identification Number. 092, F.S. The term “Employer Identification Number (EIN)” refers to a nine digit number, which are assigned to business entities by the Internal Revenue system https://www.irs.gov/Businesses/Small-Businesses-&-Self-Employed/Apply-for-an-Employer-Identification-Number-%28EIN%29-Online Another name for the Employer Identification Number (EIN) is the Federal Tax Identification Number (FEIN). This number is mandatory for all business that has a bank account, even if it is a single member. Such businesses include: sole proprietors, partnerships, limited liability companies and corporations. It’s also compulsory for employers to have a unique federal employer’s identification number. The federal employer’s identification number, which is normally issued by the Internal Revenue Service, must be boldly written on the license application. On the other hand, self- employed entrepreneurs that have no employee are required to submit an affidavit to that effect along with their application. Form HSMV 86056 can be downloaded from HSMV86056: http://www.flhsmv.gov/dmv/forms/BFO/86056.pdf (the Department’s website). This is the right form that must be filled by anybody applying for a License as a Motor Vehicle, Mobile Home, or Recreational Vehicle Dealer.
  • Sales Tax Number. 15C-7.004, FAC. Those applying for motor vehicle dealer license are required to indicate their sales tax number on their application form. This is in accordance to rule 12A-1.066. The sale tax number can be obtained from the state Department of Revenue Link https://taxapps.state.fl.us/IRegistration/ see Unit 10. Sale tax numbers are usually issued per each location of the dealership. It is the responsibility of the dealer, to collect sales tax on each sale and subsequently remit it to Department of Revenue
  • Two positive print photographs of the proposed licensed place of business. The photographs shall be a minimum of 3” × 4” in size. One photograph shall depict the exterior of the dealership from a distance to clearly show two (2) sides of the building, one side of which shall be the public entrance into the dealership. The second photograph shall show the remaining sides of the building which will house the dealership. At least one of the photographs must reflect the area to be used for display of vehicles/units offered for sale, if display space is required
  • Photos of a permanent sign identifying the dealership at its place of business. Such sign shall clearly identify the dealership and shall use lettering or other graphic representation of sufficient size and color so as to be visible and readable at a distance of 50 yards from the public right-of-way serving the dealership
  • Photos of the hours of operation in a clear manner at or near the main entrance to the dealership office on a placard, sign or by other durable means and shall maintain the posted hours of operation.
